ACKCSC Charitable Trust Grant Application Information

 

There are now 4 separate forms that must be completed / submitted at various times throughout the life of the process. Additionally, we have created parallel instruction documents to assist you in completing the forms. These forms are:

          1. Grant Proposal (Cover Sheet and Signature Page)
          2. Grant Application
          3. Grant Progress Report
          4. Grant Final Report

When comparing the “Form” to the ‘Instruction” document you will find that they are identical with one exception. The “Instruction” document has directions and/or examples of how to complete the form; what the review committee will be looking for to help them understand your proposal. The “Form” has the same questions, but there is space after each for you to complete with your own answer. All Forms and Instruction documents can be printed if you desire to do so.

We now provide the ability to complete your application and all project reports on-line; we encourage you to use this means to submit your documentation. The file naming convention should be as follows:university name_your initials_year.file extension (for example, UGA_HEJ_2016.pdf or Cornell_HEJ_2016.pdf)

In order to use electronic submission you will need to use "Adobe Acrobat Reader DC" this is the latest version of this application.  There are different means to gain access to this version if you do not have it already.   The "internet browser" that you use determines the process by which you gain access to this version of Adobe Reader.

        • Firefox - Once you have selected and opened the 'Form' .pdf icon that you wish to complete, you should see a set of icons up at the top right of the page.
            • Select the third icon that looks like a page the "download" icon.
            • You will get a pop-up window that asks you what Firefox should do with this file. 
            • Select the 'Open with Adobe Acrobat Reader DC.
            • You will be presented with a new window containing the document in Adobe Acrobat Reader DC.
            • Using the vertical scroll bar on the far right, scroll down until you find the "Sign and Fill" icon which you must select.
            • You will again be presented a new window containing the document in a fillable version.  You will know this because you pointer will turn into a vertical text line with the letters "Ab" appearing.
            • Select the location where you wish to start entering information.  Once you have successfully anchored the spot you should see "type text here" in a box.
            • You should be good to start completing the form.
        • Internet Explorer - Once you have selected and opened the 'Form' .pdf icon that you wish to complete, you should see a gray tool bar down at the bottom of you screen. (You may have to move your pointer a lttle to get the bar to appear.)
            • Select the last icon on the right (Adobe icon), you will be taken to the Adobe Acrobat Document Reader DC screen.
            • You will be presented with a new window containing the document in Adobe Acrobat Reader DC.
            • Using the vertical scroll bar on the far right, scroll down until you find the "Sign and Fill" icon which you must select.
            • You will again be presented a new window containing the document in a fillable version.  You will know this because you pointer will turn into a vertical text line with the letters "Ab" appearing.
            • Select the location where you wish to start entering information.  Once you have successfully anchored the spot you should see "type text here" in a box.
            • You should be good to start completing the form.

All electronic submissions should be sent to: This email address is being protected from spambots. You need JavaScript enabled to view it.. Make the “Subject” of each email ‘ACKCSC-CT GRANT XXXXXXX’ (name of form that you are submitting e.g. Proposal, Application, Progress Report, etc.).  Please make sure you get confirmation that your file has been delivered. We would recommend that you “copy” yourself on this submission as well.

Should you prefer to use a mail or package system versus email we will continue accepting this method as well. Your packet must include eight (8) copies of the application, bound or stapled to keep the pages in order. You can send your package to:

Grant Proposals are reviewed by the Board of Trustees once a year at a meeting held during our National Specialty; at this time funding decisions are made. Our National Specialty occurs sometime between April 1st and May 31st; you can look on the American Cavalier King Charles Spaniel Club’s (ackcsc.org) website to find out the exact date for the current and the upcoming year.
